Elation Passport is the patient portal used by Kidan Medical. It's a secure website and companion mobile app where you can message our office, see your upcoming appointments, complete forms ahead of a visit, and review your health information, all without waiting on hold.
Patient Passport accounts are set up by invitation. If you're a new patient, we'll send you an invitation to create your account around your first visit. If you haven't received one, call us at 317-760-5319 and we'll get it sent over.
Once you have an account, sign in any time at app.elationpassport.com or through the mobile app.
Yes, one login can give you access to more than one person's chart, for example, a spouse or a child. This is called proxy access, and it's set up by our office, not something you can add yourself in the app. Call us at 317-760-5319 and we'll link the accounts for you.
Once it's set up, signing in will ask you to confirm whether "This is me" or "I manage their care." From there, use the Switch Patient menu in the app to move between the charts you have access to.
For a parent or guardian managing a child's account, there's no automatic cutoff date built into the portal, when and how that access changes as a child gets older is something we can talk through with you directly. Just ask us.
If you're on your phone, the free "Elation Passport for Patients" app is the easiest way to stay connected. It's available for both iPhone and Android, and supports fingerprint sign-in.
