What to Expect

Our care coordinator will reach out first to schedule a short welcome call with you, by video. We'll ask what you want from your care, walk through insurance or Direct Primary Care setup, and then get your visit on the calendar.
You'll receive a confirmation email with date, time, and any prep instructions.
Have a valid photo ID, insurance card, and a list of current medications ready for your visit.
For telehealth, click your video link a few minutes early to test your camera and mic. For home visits, we'll text you when the provider is on the way.
Medical emergency? This form is not monitored in real time. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room. For a mental health or suicide crisis, call or text 988.
New patients: We'll send you a patient intake form to complete before your visit so we can make the most of your time with us.
Cancellations: Please give us at least 24 hours' notice if you need to cancel or reschedule, so we can offer the time to another patient. Your first missed appointment is a courtesy with no charge. After that, a $30 fee may apply. This fee is never billed to insurance, is not charged to Medicare or Indiana Medicaid patients, and is waived for emergencies, hospitalization, bereavement, or transportation problems: just let us know.
