Chronic Care Management & Remote Monitoring at Home

Diabetes, hypertension, heart failure, and COPD need steady attention between visits, not a rushed appointment every few months. We manage these conditions closely, with remote monitoring so small changes get caught early.

Closer Follow-Up Catches Small Changes Early

A rushed appointment every few months misses the small changes that lead to hospitalizations. Kidan Medical manages chronic conditions with regular in-home and telehealth visits, plus monitoring between visits so problems get caught before they become emergencies.

How We Help

  • Regular in-home and telehealth visits
  • Chronic care management coordination between visits
  • Remote patient monitoring: take-home blood pressure, weight, and glucose devices
  • In-home labs and diagnostics
  • Medication management
  • Integrated behavioral health

What Remote Monitoring Actually Involves

You're set up with simple connected devices, for example a blood-pressure cuff, a scale, or a glucose monitor. Readings are reviewed by our team between visits, and your provider follows up and adjusts your care when a reading moves out of range, no extra trip required.

Why In-Home, Why Us

Closer follow-up, monitoring between visits, and diagnostics in the home mean problems get caught earlier, before they turn into a hospital trip.

A 2025 Medicare claims study of a remote patient monitoring program found a reduction in total cost of care of about $1,302 per patient per year, driven mainly by lower inpatient costs.1
